Call Program Lab Set-up
- ProLab Consulting, LLC’s Call Program Lab Set-up consulting option provides assistance via phone, fax and/or e-mail to make sure your lab is set up correctly from CLIA applications, guidance with policy and procedures, to preparation for survey. The Call Program Lab Set-up consulting option will ensure your practice’s laboratory will be up and running with ease.
The Call Program Lab Set-up option includes:
- CLIA licensure or upgrade applications
- State-specific licensure
- Enrollment in proficiency testing
- Evaluation of laboratory director and personnel qualifications
- Enrollment in laboratory director course
- Enrollment with accreditation organization if desired
- Compilation of general laboratory policy and procedure manual
- Compilation of assay-specific policy
- Compilation of a complete quality assessment plan
- General laboratory safety policy
- Data reduction in validation of analyzers
- All logs for reagent inventory, temperature, QC and patient testing
- General guidance for proper CPT coding
- Preparation for CLIA or accreditation organization survey and inspection process
